Care Coordination

Care Coordination in Denver

We collaborate with healthcare providers and community resources so your care fits together instead of sitting in separate boxes.

What care coordination is

Our staff collaborates with healthcare providers, counselors, and community resources to ensure patients receive well-rounded, coordinated care. We help connect individuals with additional services such as primary care, mental health support, and social services.

Opioid treatment rarely stands alone. Coordination means the people treating the rest of your health know what your treatment plan is, and you are not the one left holding every thread.

Who it is for

  • Patients who have other health conditions being treated elsewhere
  • People who need mental health support alongside opioid treatment
  • Patients who are not currently connected to a primary care provider
  • Anyone who needs help finding social services in the Denver area
